The Florida Discount Drug Card is designed to lower the cost of prescriptions for all Florida residents. Please select an option below to sign up:

		I am a Florida resident and would like to participate in the program.
OR
		I am a Florida resident and I am age 60 to 64, without prescription drug coverage, and do not belong to a Medicare Part D plan.
		I am a Florida resident and I am under age 60, without prescription drug coverage, and with an annual family income of less than 300% of the Federal Poverty level. Qualifying incomes include those below the following: $34,470 (for an individual) $46,530 (for a family of two) $70,650 (for a family of four) Click here for income limits for larger families
Note: Florida residents who qualify based on age or income status are eligible to receive additional savings.
